T.D. Medical College was established in 1963 as Tirumala Devaswom Medical College — the first medical college set up in the cooperative sector of India. The first batch of 50 MBBS students began classes in August 1963. The Government of Kerala subsequently took over the institution and via GO (MS).263/73/HD dated 23 October 1973 upgraded the Alappuzha District Hospital as the official teaching hospital.

TDMC offers 175 MBBS seats per year. Of these, 15% (approximately 26 seats) are under the All India Quota (AIQ) managed by MCC/DGHS, and 85% (approximately 149 seats) are under Kerala State Quota via CEE Kerala. As a government college, there are no management or NRI quota seats.

The annual tuition fee for MBBS at Government TDMC Alappuzha is just ₹23,150 per year, making the total course fee approximately ₹1.16 lakh for 4.5 years — among the most affordable medical degrees in India. This is the highly subsidised government tariff. Additional nominal university and miscellaneous fees apply.

TDMC Alappuzha offers 80 MD/MS postgraduate seats across multiple specialties including General Medicine, Paediatrics, Anaesthesiology, Pathology, Radiodiagnosis, General Surgery, Obstetrics & Gynaecology, and others. It also offers DM/MCh super-specialty courses, along with a PG Diploma in Obstetrics & Gynaecology (2 seats). A total of 42 courses including Nursing, B.Pharm, D.Pharm, and allied health diploma programmes are offered.
