NEIGRIHMS has an annual MBBS intake of 100 seats (expanded from 50 in the 2024 batch). The original 50-seat distribution is: Meghalaya (14), Nagaland (10), NE Open for all 8 NE states including Sikkim (18 seats — filled via NEET UG), and All India AIQ via MCC (8 seats). The additional 50 expansion seats are distributed proportionally across these categories.

NEIGRIHMS offers heavily subsidized MBBS education as a central government premier institute. Annual tuition fee is approximately ₹16,600. Admission fee is approximately ₹11,000, and a security deposit of approximately ₹20,000 is payable.
