SLIMS was established in 2006 in Kudupakkam, Puducherry. It is affiliated with the Bharath Institute of Higher Education and Research (BIHER), a deemed university in Chennai, and is recognized by the NMC and Ministry of Health & Family Welfare, Government of India. BIHER has been ranked among India's Top 150 Medical Colleges by NIRF 2022.

SLIMS has 250 MBBS seats annually for AY 2025-26, as recognized under the NMC Act, 2019. This is an expansion from the earlier 150 seats. All 250 seats are filled through MCC Deemed University counselling — 100% centralized and merit-based.

The annual MBBS tuition fee under Management Quota is ₹25,00,000 per year (total ₹1,12,50,000 for 4.5 years). NRI Quota annual fee is USD 60,000 per year. Fees are subject to revision by the State Level Fee Determination Committee, Puducherry. Hostel fees are separate and non-refundable once paid.
