AIIMS Madurai currently operates from a temporary campus at Government Rajaji Medical College, Ramanathapuram, Tamil Nadu, while its permanent campus is being developed at Thoppur, Austinpatti, near Madurai. Students will move to the permanent campus once construction is complete.

The planned AIIMS Madurai teaching hospital at the permanent campus is designed for over 750 beds. As the institute is still developing its full clinical infrastructure, MBBS students currently gain clinical exposure through the temporary campus's association with Government Rajaji Medical College facilities in the region.

For 2024 MBBS admissions (Round 1), the NEET UG closing rank for the General category was around 4,531, one of the higher closing ranks among AIIMS institutes, reflecting its status as one of the newer campuses. Admission is via MCC AIQ counselling only — there is no state quota.
