T.D. Medical College was established in 1963 as Tirumala Devaswom Medical College — the first medical college set up in the cooperative sector of India. The first batch of 50 MBBS students began classes in August 1963. The Government of Kerala subsequently took over the institution and via GO (MS).263/73/HD dated 23 October 1973 upgraded the Alappuzha District Hospital as the official teaching hospital.
