TRRIMS was established in 2019 by Sri T. Ram Mohan Reddy under the TRR Group of Institutions. The TRR group was originally founded in 2001 with the engineering college at the same campus (TRR Nagar, Inole, Patancheru). The medical college started MBBS admissions from AY 2019-20.

150 MBBS seats per year. Seat distribution: 50% Cat A (Competent Authority / State Quota) = 75 seats via KNRUHS; 35% Cat B (Management Quota) = 52 seats; 15% Cat C (NRI Quota) = 23 seats.

State quota (Cat A): ₹60,000 per year. Management quota (Cat B): ₹11,55,000 per year. NRI quota (Cat C): ₹23,10,000 per year. Hostel, mess, and caution deposit are additional to tuition. Fees are subject to revision by State Level Fee Determination Committee.
