Marg, Nagpada-Mumbai Central, Off Jijabhoy Road, Mumbai — 400008 → Step 7: Fee via DD from Nationalized/Scheduled Commercial Bank in favour of Dean/Principal. Documents: NEET scorecard + admit card, HSC/12th + SLC, Class 10 marksheet, Caste Certificate + Caste Verification (Annexure-3 per MUHS), OBC-NCL Certificate, EWS Certificate, Photo ID (Passport/PAN/Aadhaar/Driving License — self-attested), Birth Certificate (if DOB not on HSC), Medical Fitness Certificate (Annexure-H), Online Counselling Registration Certificate, Sponsorship Certificate (in-service), 8 passport-size photos.
